Each episode is told through the narrative and eyes of main character, John 'J.D.' Dorian. Along with internal monologues and surreal scenario enactments, J.D. witnesses the challenges and struggles of his fellow interns and doctors, each episode with a moralistic ending.

J.D. starts of at Sacred Heart Hospital as an intern, along with his best mate Turk, and fellow intern, Elliot Reid.

They all become close friends with hospital nurse, Carla Espinosa, with whom Turk later has a long-term relationship with.

J.D. is rarely seen without Turk, his closest friend since college and Med school. Their friendship is often described as more of a marriage, with J.D. feeling vulnerable when Turk spends time with Carla.

J.D. also has an almost unhealthy infatuation with his apparent mentor, Dr. Perry Cox.

Perry is the doctor J.D. most aspires to be, despite being consistently put down by Perry through a series of name calling and his unapproachable persona. J.D. also has a habit of daydreaming, creating surreal scenarios that he wonders about throughout the day, and these are often a source of annoyance to his friends who wonder what he is thinking.

Turk is a very competitive person, which helped drive him to become a surgeon at Sacred Heart. Having initially refused, Carla agrees to go on a date with Turk, and they quickly become a serious item, and later marry and have a child.

Although Carla supports Turk and J.D.'s close friendship, she wonders if Turk will ever love her the way he loves J.D.

Turk also struggles with his faith, as a doctor he witnesses many untimely deaths. Although he is very self-assured, we understand that nothing is more important to Turk that being a good doctor, friend, husband and father.

Elliot is doctor who struggles with many personal issues, including crippling low-self esteem, and the pressure of proving to be a good female doctor in a seemingly misogynistic profession.

Elliot and J.D. have often pushed the boundaries between friendship and romance, but remain close friends throughout.

Elliot finds most social scenarios highly uncomfortable, often putting her foot in her mouth or completely embarrassing herself.

Carla is one of Sacred Heart's most capable nurses, but is prone to over-reacting and telling others how to live their lives. Unlike anyone else in the hospital, Carla and Perry have a mutual respect for each other, as she seems to be the only one who 'gets him'.

Her friendship with Elliot was rocky in the early stages, although at Carla's wedding she admits she is closer to Elliot than any of her sisters.

Dr. Perry Cox has a quick cruel wit, and keeps up his 'bad boy' image through a series of illogical and self-destructive decisions. His macho, superior persona is highlighted through giving J.D. girl's names, and by calling Elliot 'Barbie'.

He also lounges around his apartment topless, showing that he is physically fit.

Season One
The characters face professional pressures, as well as romance and friendship issues.

Turk starts dating Carla, J.D. gets a crush on Elliot, Perry becomes a reluctant mentor, and the janitor's feud starts with J.D.

Season Two

Now all residents, they are faced with even more personal and professional dilemmas.

Perry has a sexual relationship with ex-wife Jordan, Elliot gets a new boyfriend, Turk proposes to Carla, and J.D. struggles with financial and romantic problems.

Season Three

J.D. becomes attracted to Elliot once again after her image transformation, but instead starts a relationship with Jordan's sister.

Elliot also starts a relationship with an ex-boyfriend. Turk and Carla are planning their wedding, and Perry and Jordan seem happy together and with their new baby boy.

Season Four

Turk and Carla return from their honeymoon, but already their relationship is under pressure as Carla tries to change her new husband.

Elliot is mad at J.D. for breaking her heart, a situation worsened when she starts dating his brother. J.D. starts dating attractive psychiatrist, Dr. Molly Clock, who also becomes Elliot's new mentor.

Season Five

J.D. is now attending on the same level at Dr. Cox, and Elliot has taken a new fellowship at another hospital.

Turk is having doubts about him and Carla trying for a baby, and some new interns arrive at Sacred Heart, notably Elliot's future boyfriend, Keith Dudemeister.

Season Six

J.D. becomes an expecting father as his girlfriend, Dr. Kim Briggs, becomes pregnant. Carla and also become parents when their baby girl is born.

Despite still harbouring feelings for each other, Elliot tries to get over J.D. by planning to marry Keith, and issues regarding death, leadership and fate are questioned.

Season Seven

J.D. and Elliot work on their individual relationships, and Chief of Medicine, Dr. Kelso, is forced to retire when his age is revealed.
